Welcome to our office!  Our goal is to make your dental care a pleasant experience and to assist you in understanding your dental needs.  After your initial examination, we will discuss your treatment, any options and give you a written estimate for each procedure.  If you have dental in surname, we will inform you of your benefits and any copayments that may be due at the time of treatment.  We cannot guarantee benefits as each plan has different benefits and limitation.

We will attempt to confirm all scheduled appointments the day before treatment.  Please understand if we are unable to reach you.  We ask that you help us by noting the day, date and time of your next visit (it is also listed on your walkout statement).  If you are unable to make an appointment, we appreciate a full workday notice so we may schedule another patient that may be waiting for treatment.  If we do not receive a full workday notice, we may charge a broken appointment fee of up to $200.00.

We strive to stay on schedule and complete your treatment in a timely manner.  However, emergencies do occur and must be cared for immediately.  Please bear with ou during these times and we will do our best to see you as quickly as possible.  We do ask that you arrive on time for your appointment.  If you are on a tight schedule, please inform our receptionist so we can alert the staff.  Please do not hesitate to ask questions, we can help you!

We require all children under the age of 18 be accompanied by a parent or legal guardian.  During the time the patient is in our office, we request the parent/guardian be present.  We appreciate your assistance in treating your child. 

We are always looking for ways to better serve our patients, so let us know if there is anything that we can do to make your visit more comfortable.

I have read and understand the above information.  I understand that any estimates given by any staff members may not be accurate due to changes in treatment/or insurance limitations and coverage (for instance, during cleaning we might detect more decay under fillings/crowns/roots).  Most of the time, small decay can not be detected on x-rays and thin decay that is not deep but wide will often not be detected as well.  I understand that I am solely responsible for all charges incurred for services performed.  I also agree to pay any remaining balance not paid by my insurance(s).
